In order to use this form to request disclosure, you must have received either of the following documents by mail or email from the Court Office:

You must have a court date before you can request disclosure.
Your court date can be located on your Notice of Trial or on your Early Resolution Meeting Notice.
If you do not have a court date, contact the provincial offences office at 519-271-0531 x249.
