Request for Disclosure

In order to use this form to request disclosure, you must have received either of the following documents by mail or email from the Court Office:

• Early Resolution Meeting Notice (link opens in a new window)
• Notice of Trial (link opens in a new window)

All of the information required below, will be on these documents. You must have a court date before you can request disclosure. 

Your court date can be located on your Notice of Trial or on your Early Resolution Meeting Notice.
